BluePath Health works with a broad spectrum of public and private organizations to increase efficiencies in the health care system and improve health and social services access for the most vulnerable populations. We are an approved vendor on the CalAIM Providing Access and Transforming Health (PATH) TA Marketplace as well as a PATH Collaborative Planning and Implementation (CPI) facilitator. Additionally, we have deep expertise building and running coalitions of diverse stakeholders collaborating to advance a common agenda.

CalAIM Tools and Resources

Alameda CalAIM PATH Collaborative

BluePath Health is thrilled to be the facilitator for the Alameda PATH Collaborative. The Collaborative meets monthly to discuss CalAIM implementation and support the rollout of CalAIM Enhanced Care Management (ECM) and Community Supports. Health plans, county leaders, community-based organizations, hospitals, clinics, and other CalAIM stakeholders are encouraged to join. The Medi-Cal managed care plans in Alameda County include Alameda Alliance for Health and Anthem Blue Cross Partnership Plan.

Tri-Counties CalAIM PATH Collaborative

BluePath Health also facilitates the Tri-Counties PATH Collaborative. The Collaborative comprises San Luis Obispo, Santa Barbara, and Ventura Counties. The collaborative meets monthly to discuss CalAIM implementation and support the rollout of Enhanced Care Management (ECM) and Community Supports. Health plans, county leaders, community-based organizations, hospitals, clinics, and other CalAIM stakeholders are encouraged to join. The Medi-Cal managed care plans in this region include CenCal Health (San Luis Obispo, Santa Barbara) and Gold Coast Health Plan (Ventura). 

PATH TA Marketplace

In 2023, BluePath Health was selected by the Department of Health Care Services as a CalAIM PATH TA Marketplace vendor. The TA Marketplace is available to providers, community-based organizations, counties, and other entities implementing Enhanced Care Management (ECM) and Community Supports. BluePath Health will offer TA to eligible organizations in the domains of Building Data Capacity, Supporting Cross-Sector Partnerships, Engaging in CalAIM through Medi-Cal Managed Care, and Promoting Health Equity.
