BluePath Health shares details on the important programs supported through the TA Marketplace.
The California Technical Assistance (TA) Marketplace is a vital resource for organizations striving to enhance community health and human services across the state in support of CalAIM. Part of California’s Providing Access and Transforming Health (PATH) initiative, the program is designed to enhance the capacity and infrastructure of providers participating in the Medi-Cal delivery system. At no cost to these providers, the TA Marketplace offers access to a platform of qualified vendors, including BluePath, to support and deliver key projects—helping make whole person care across California a reality.
Here are just a few of the projects that BluePath has supported through the TA Marketplace.
Connecting Orange County’s Most Vulnerable Through 2-1-1 OC
Since its founding in 1984, 2-1-1 Orange County (211OC) has been instrumental in linking residents with critical social services, including housing, job placement, food, and health insurance. BluePath Health and Orange County United Way are currently focused on aligning data sharing capabilities and coordination to best support CalAIM services, such as Enhanced Care Management (ECM) and Community Supports (CS).
Fighting Hunger with Alameda County Community Food Bank
The Alameda County Community Food Bank (ACCFB) envisions a hunger-free community where no resident has to worry about their next meal. To help advance this mission, BluePath Health supported ACCFB with a Phase I Readiness Assessment and workflow design, ensuring efficient service delivery and improved resource management.
Advancing Senior Care with Omatochi
Omatochi is revolutionizing in-home senior care by prioritizing independence and holistic wellness. BluePath Health has supported Omatochi’s CalAIM outreach efforts by facilitating community events, and strengthening referral networks. This includes conducting a comprehensive Community Services Capacity Assessment through 20+ local provider interviews; managing and facilitating three Senior Wellness Fairs and a Care Workforce Summit; and promoting career pathways for caregivers while addressing healthcare access disparities and social isolation.
Supporting Fresno Probation Department’s Justice-Involved Initiatives
The Fresno Probation Department is committed to community protection while offering intervention programs for justice-involved youth. To align with CalAIM’s Justice-Involved Initiative, BluePath Health is conducting a landscape analysis of workflows and systems, identifying gaps relative to Enhanced Care Management (ECM) requirements, and coordinating with external stakeholders to support Fresno’s work to become an ECM provider.
Enhancing Healthcare with MedZed
MedZed partners with health plans to reach high-risk members who have complex healthcare needs. Since 2023, BluePath Health has been collaborating with MedZed to select and implement electronic health record (EHR) and population health management systems, enabling MedZed to increase efficiency and improve care delivery.
Strengthening Data Analytics for the California Food is Medicine Coalition
The California Food is Medicine Coalition (CalFIMC) provides medically tailored meals to low-income individuals with chronic illnesses. BluePath Health is spearheading a major initiative for CalFIMC to bolster its analytic capabilities. Components have included identifying an analytics vendor, identifying and contracting with QHIOs and other health information networks that are critical partners for CalFIMC organizations, and building critical data governance and HIPAA compliance capabilities to enable this work.
The California TA Marketplace continues to drive high-impact community health initiatives. By leveraging technical assistance, data-sharing strategies, and collaborative engagement, organizations statewide are better equipped to address social drivers of health and enhance service delivery in support of whole person care.
